As an approved provider of Home Care Services as part of the Commonwealth Home Care Packages program, Clayton Church Homes can help you to maintain the independence you enjoy in your own home for as long as you wish no matter what your needs are.

If you are not eligible for a government-subsidised Home Care Package or are waiting for your Package to be approved through My Aged Care, we can support you without the need for a doctor’s referral on a Fee-for-Service basis.

Our Home Care Services include:

  • Personal care: Assistance with showering, dressing, mobility, and more.
  • Daily Living Support: Help with meal preparation and cooking, toileting, laundry, shopping, and transportation.
  • Health Management: Including nursing services, allied health therapy, medication support, continence, and skin integrity management.
  • Social and Emotional Support: Respite care, social activities, emotional support, and cognitive care.
  • Home Maintenance: Gardening, safety modifications, cleaning, and transport services.

How It Works:

  • Customised Care Plan: We work with you to create a care plan addressing your goals and needs, including services from third-party providers if needed.
  • Flexible Care Levels: We cater to all Home Care Package Levels (Levels 1 – 4, as determined by your ACAT assessment).
  • Care Management: Ongoing monitoring and adjustments to ensure the care plan continues to meet your needs.
